2. 2. Fill out the form and send it in to the address listed below or send an e-mail with the information to marketing@messinahof.com or fax to 979-778-1729.
3. 3. Include along with the form, a copy of your non-profit tax information.

note: Please be sure to list the address you want the donation to be sent to if it is different from the contact address you have listed on the form.
